The following resources are required:

Questionnaire-paper, computer or web based
Some studies will use interviewers to administer the PAQ who will need training
Cover letter, or detailed instructions on the cover
Additional socio-demographic information and basic anthropometry (weight and height).
Instructions for completion and return regardless of media
For mailed questionnaires a pre-paid stamped address envelope
Standard operating procedures for interviewers, coding and analysis
Analysis program or database and use of algorithms for conversion to energy expenditure
